Modeling Contract

This agreement is entered between _____________________________________________(Model), and Chelas Montanye and Joshua Myers (Artist/Photographers) Concerning their photo shoot on______________, at ____________________________________________ _____________ _____ _________ (location)

For value recieved the artist/photographers, his/her heirs, legal repsentatives and those acting with the artist/photographers' permission are granted the right to use the images of the model for the artist/photographers' portfolio, sales, exhibitions, for submission to stock agencies, and for advertisments that promote the artist/photographers' skills. The artist/photographers have the right to retouch enhance and/or remove blemishes on images of the model. The artist/photographers are granted permission to display the images for the above mentioned purposes on the internet. The artist/photographers are permitted to identify the model by ___________________. Notwithstanding the above, model retains ownership of his likeness.

The artist/photographers, his/her heirs, legal repsentatives and those acting with the artist/photographers' permission agree/s not to use or allow images of the model in whole or in part to be used in ways that would slander, attack or deform the character or the person of the model.

The model, his/her heirs, legal repsentatives and those acting with the model's permission are granted the right to use of images for their portfolio and to promote themselves, but are not granted permission to edit or sell any images provided without written consent from the artist/photographers.

The Model will provide the the artist/photographers with a $60 kit fee for body paints. In return the model, his/her heirs, legal repsentatives and those acting with the model's permission acknowledge that they will receive all edited and unedited digital images from the shoot.

LIABILITY STATEMENT

Chelas uses FDA approved cosmetic grade face paints.

Chelas’ preferred paint is Paradise Make-up AQ. http://www.mehron.com . These paints contain ingredients such as aloe and chamomile and are safe and gentle to the skin. They require no special removers and wash off easily with just soap and water. Chelas is not liable for allergic reactions to the paints. People with skin allergies or sensitive skin should not be painted, or have a patch test prior to the body painting.  For safety reasons, Chelas will not paint anyone who does not want to be painted. For sanitary reasons, Chelas will not paint anyone who is, or appears to be sick or suffering from cold sores, conjunctivitis, ringworm, sunburn, eczema, any infectious skin disease, or open wounds. These determinations are at the artist’s discretion. Chelas will use reasonable care, but is not responsible for damage to clothing or property.
